Overview
The Hospice, Palliative Care, and Homecare Social Worker functions as a member of the interdisciplinary team for Hospice, Palliative Care, and Homecare. The Social Worker provides clinical social work services to patients and their families enrolled in Hospice, Home-Based Palliative Care program and to select patients and their families receiving services from Homecare. The Social Worker provides initial psychosocial evaluations, psychosocial counseling and intervention, direct casework services, grief/bereavement services, and community education, outreach and referrals. The social worker also acts as the Hospice Volunteer Coordinator and/or Hospice Bereavement Coordinator. The Social Worker also provides consultation, education and training to home care staff regarding psychosocial issues that impact the delivery of optimal home care services. |   | Schedule:  | Monday-Friday with one day off during the week  | 8am-5pm
