Overview
The Hospice, Palliative Care, and Homecare Social Worker functions as a member of the interdisciplinary team for Hospice, Palliative Care, and Homecare. The Social Worker provides clinical social work services to patients and their families enrolled in Hospice, Home-Based Palliative Care program and to select patients and their families receiving services from Homecare. The Social Worker provides initial psychosocial evaluations, psychosocial counseling and intervention, direct casework services, grief/bereavement services, and community education, outreach and referrals.
