Overview
The Family Based Intensive Treatment (FBIT) Program is an intensive home- and community-based treatment service working with youth and family to address behavioral health difficulties in the home, school and community. Typically, the youth displays impairment in at least two of these arenas. FBIT targets a population of youth with significant psychiatric needs in order to increase functioning and sustainability within the family system. The FBIT Therapist will utilize the Intensive Home Based Treatment model. The goal of the program is to stabilize mental health concerns, and safely maintain the youth in the least restrictive, most normative environment. The Behavioral Health Therapist will provide both therapy and case management services in the community, while working closely with community stakeholders. Treatment is intensive (3-5 sessions/week) and time limited (average of 4 to 6 months). The program provides services to clients between the ages of 4-18 exhibiting a wide range of psychopathology. 24/7 crisis support is provided. This position offers weekly individual supervision, as well as weekly consultation provided by Center for Innovative Practices.

Overview
The Hospice, Palliative Care, and Homecare Social Worker functions as a member of the interdisciplinary team for Hospice, Palliative Care, and Homecare. The Social Worker provides clinical social work services to patients and their families enrolled in Hospice, Home-Based Palliative Care program and to select patients and their families receiving services from Homecare. The Social Worker provides initial psychosocial evaluations, psychosocial counseling and intervention, direct casework services, grief/bereavement services, and community education, outreach and referrals. The social worker also acts as the Hospice Volunteer Coordinator and/or Hospice Bereavement Coordinator. School-based Health Services increases access to health care at multiple Central Ohio Schools, providing primary care and a variety of other services to students and families. The Clinical Medical Social Worker embedded in the School Based Health Clinics are a part of a multidisciplinary team to improve health outcomes.
