Overview

The Behavioral Health (BH) Inpatient Discharge Planner is responsible for providing care coordination and disposition planning to children, adolescents, and their families while admitted to the inpatient psychiatric unit.  These responsibilities are comprised of case management procedures, discharge planning, and clinical care coordination.  This individual is expected to utilize best practice standards that are informed by the professional literature.  The Discharge Planner will possess the skills to provide family education and intervention around care coordination, case management, and disposition planning in an inpatient behavioral health setting.  This individual will provide culturally appropriate interactions and interventions across all professional and clinical settings.

Responsibilities

The Behavioral Health (BH) Inpatient Discharge Planner implements and supports the philosophy, mission, values, standards, policies, and procedures established by Nationwide Children’s Hospital Behavioral Health Crisis Services and appropriate Ohio Licensure requirements.

 

The Behavioral Health (BH) Inpatient Discharge Planner assists and facilitates coordination of care for patients admitted to the inpatient psychiatric unit during admission and post-discharge and other responsibilities as outlined below:

•            Assists with phone call management, including post discharge follow-up calls where applicable.

•            Administers, completes, and processes intake and discharge paperwork for all patients referred to the inpatient psychiatric unit including distribution and collection of discharge surveys and POC surveys.

•            Supports discharge follow up care by placing referrals into EPIC and scheduling discharge follow up treatment appointments.

•            Supports care coordination activities including scheduling, data entry, report generation, distribution, and communication with internal and outside entities.

•            Remains knowledgeable of internal and external agencies waitlists, accepted insurance, and referral process.

•            Coordinates, mobilizes, and schedules, as needed, community resources to enhance client care (i.e. interpreters for treatment sessions, financial counselors for family support, transportation, and completes necessary referrals for Ronald McDonald stay).

•            Collaborates consistently with the inter-disciplinary team.

•            Attends routine staff meetings & client treatment rounds.

•            Utilizes EPIC for documentation of care coordination activities in accordance with policy standards.

•            Coordinates client care with other service providers to ensure continuity of care.

•            Assists with data gathering and information tracking as necessary for process improvement.

•            Meets regularly with identified supervisor.

•            Demonstrates willingness to seek/accept direct supervision, as well as, respect productive feedback from colleagues and others.

•            Attends in-services trainings, peer review and other interagency and supervisory activities.

•            Strives to maintain positive patient satisfaction.

•            Practices within professional areas of expertise, maintain applicable licensure. Attends in-services trainings, peer review and other interagency and supervisory activities

•            Performs other duties as assigned by supervisor or other designee.

REPORTING RELATIONSHIPS

Reports to Behavioral Health (BH) Inpatient Clinical Lead Supervisor

Qualifications

KNOWLEDGE , SKILLS AND ABILITIES REQUIRED

  • Bachelor’s degree from an accredited university or professional school of social work, counseling, psychology or a related field;
  • Demonstrated skills and abilities in providing and managing behavioral healthcare to children, adolescent and families;
  • Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients, families, peers and community representatives;
  • Ability to work with the hospital’s electronic medical records system.
  • Keyboarding/typing, data entry skills; and computerized scheduling experience preferred.
  • Familiarity with spreadsheets, data tracking preferred
  • Ability to work and problem-solve with minimal supervision; ability to adapt to change positively in work flow and process.

MINIMUM PHYSICAL REQUIREMENTS

 

•            Constantly must hear, see, speak, and be physically mobile

•            Demonstrate ability to pass TCI crisis management trainings once trained
