0.9 FTE, Average of 36 hours per week (working 32 hours one week & 40 hours the nex week) 

Day Shift (8am-5pm), Monday-Friday

The Cardiopulmonary Rehabilitation Coordinator is responsible for assisting patients and families in the coordination and implementation of their cardiopulmonary rehabilitation plan of care. In collaboration with other members of the team, the Cardiopulmonary Rehabilitation Coordinator ensures high standards of care, pertinent clinical education programs and cost-effective patient management plans. They foster effective communication among health care professionals and serves as a liaison to various community organizations and corporations. The Cardiopulmonary Rehabilitation Coordinator collaborates with the Pulmonary medicine team, the Cardiac medicine team, the Clinical Therapies Department, and other administrators of affiliated departments in the development, implementation, and evaluation of all program components.

 

The Cardiopulmonary Rehabilitation Coordinator is responsible for the operational management of the program which will include patient recruitment, scheduling of the needed physical testing, development of written individualized care plans with projected outcomes/goals, service co-ordination, payor approval and financial management. The Cardiopulmonary Rehabilitation Coordinator will be responsible for ensuring that the program meets all the certification requirements for Cardiac and Pulmonary Rehabilitation Program Certification. The Cardiopulmonary Rehabilitation Coordinator manages all pertinent clinical educational programs, patient interventions, budget items, data collection, evaluations and personnel matters to accomplish the program's goals and objectives. The Cardiopulmonary Rehabilitation Coordinator explores and secures alternative funding sources and serves as a liaison to various community organizations and corporations.

Responsibilities

1. The Cardiopulmonary Rehabilitation Coordinator implements and supports the philosophy, mission, values, standards, policies and procedures of Children?s Hospital and the Patient Care Services Division.

2. The Cardiopulmonary Rehabilitation Coordinator performs case management core functions which include:
a. screening and identification of patients
b. establishing an effective relationship with the patient and family
c. assessment of patient/family strengths and needs
d. development of a family-centered plan of care
e. procurement and coordination of services
f. monitoring and evaluation of progress

3. Facilitates the patient/family?s ability to assume responsibility for managing their own health care when appropriate by:
a. promoting return of the patient/family to the highest level of wellness possible
b. assisting patient/family to understand their disease process and their management plan
c. assisting the patient/family to identify potential risk factors and barriers to health
d. ensuring that the right services are delivered at the right site by the right provider at the right time for the right cost
e. assisting in the acquisition or verification of financial/payor/insurance information when appropriate

4. Collaborates with the Pulmonary medicine and Cardiac medicine teams to plan and develop program goals and services, and measure and evaluate program outcomes.

5. Works in collaboration with the physicians and other health team members in coordination the rehabilitation plan of care

A. Observes and monitors patient?s physical and mental status in the outpatient setting.
B. Validates patient/family understanding of the program interventions.
C. Monitors patient compliance during and after the program.
D. Provides ongoing education and support.

6. Facilitates the development of required protocols and clinical pathways to ensure the appropriate utilization of service provision and third party payor reimbursement.

7. Coordinates and mobilizes health care and community resources to meet the needs of patients and their families through referral, consultation and/or care conferences.

8. Develops and maintains relationships with appropriate community organizations targeting education and service for asthmatics.

9. Ensures collaboration with insurance and managed care companies to help obtain and/or maintain provider status and adequate reimbursement for services.

10. Acts as a resource to members of the health care team in providing patient, family, and staff with information regarding diagnosis, treatments, and therapeutic trends in specialty area.

11. Analyzes clinical trends and data to identify patterns for continuous quality improvement studies and/or research.

12. Assists with interim and final reports as required by program and/or funding sources.

13. Develops documentation system for the program to organize data collections and facilitate evaluation.

14. Reviews the requirements for Pulmonary Rehabilitation Program Certification and implements processes to meet the Certification requirements with in one year of program implementation.

15. Serves as a patient/family advocate in meeting patient/family needs.

16. Actively pursues education related to specialty area and health care market.

17. Should be able to perform duties of the Pulmonary Lab Respiratory Therapist

18. Respiratory therapists are authorized to access the unit medication rooms or bedside medication drawers in order to retrieve medications they are authorized to administer.

19. Supports and advances respiratory care practice through one or more of the following:
a. Participates as a conference speaker/presenter
b. Presents to community groups
c. Participates in professional organizations and activities
d. Participates on committees, task forces and advisory boards (department, hospital, community)
e. Participates on appropriate quality teams
f. Participates in scholarly activities
g. Publishes in journals, newsletters

20. Performs other duties as assigned.

Qualifications

KNOWLEDGE , SKILLS AND ABILITIES REQUIRED

1. Registered Respiratory Therapist credentialed through the NBRC with licensure to practice in the State of Ohio by the Ohio Respiratory Care Board.

2. Bachelor's degree or pursuing Bachelor's degree and already possessing an Associate's degree from and AMA-accredited Respiratory Care program.

3. Must possess and maintain current CPR card, and obtain and maintain PALS certification.

4. Should possess the Asthma Educator Credential (AE-C), or obtain within 6 months of hire.

5. Experience in health-related education, research, program development and management, budgeting and data management desirable.

6. 1 year supervisory experience desired.

7. Effective customer service, communication (oral, written, presentation) and interpersonal skills required for interaction with contacts.

8. Professionally appropriate analytical and organizational skills required.

MINIMUM PHYSICAL REQUIREMENTS

Must be able to see, hear, speak, read, and perform manual tasks with or without accommodation and care for oneself with little or no difficulty. Must be able to apply the principles of body mechanics in lifting or moving patients and/or equipment. Must be able to independently lift up to 50 pounds. Performance of principle duties and responsibilities may require the ability to travel.
