Behavioral Health Social Work Care Coordinator - Schrock Road Psychiatry

1.0FTE, 40 hours per week, M-F 8am - 5pm

The Care Coordinator assesses key needs, addresses local and systemic issues interfering with care, and assists families in setting appropriate goals and tracking progress toward achieving those goals. The Care Coordinator navigates the health care system on behalf of high need patients, supervises and provides care coordination, plans for education, and provides for patient safety and quality of care. The incumbent will serve as an agile problem solver on multiple levels, practicing professionally, ethically, and promoting patient advocacy.

Responsibilities

  • Screening and identification of patients (Frequently)
  • Outreach to primary care physicians, specialty physicians and other service providers to identify patients in need of care coordination (Continuously)
  •  Completes comprehensive Health Risk Assessment (Frequently)
  •  Develops family-centered plan of care (Frequently)
  •  Facilitates communication and collaboration amongst the healthcare team (Continuously)
  •  Develops patient?s/family self-management skills through education and resource provision (Continuously)
  •  Manages transitions between settings, caregivers and providers (Frequently)
  •  Provides monitoring and follow-up across the continuum of care (Continuously)
  •  Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services (Occasionally)
  • Participates in orientation and continuing education of staff and students as appropriate (Frequently)

Qualifications

MIMIMUM REQUIREMENTS:

1. Current Social Work license in Ohio required.

2. Masters Degree in social work, required.

3. LSW, LPC, (LISW or LPCC preferred)

4. Two years experience in social work required 

5. Valid driver's license & proof of auto insurance, (required to do home visits in 34 county area)

6. Working knowledge of Medicaid and other regulatory agency standards

7. Active BLS certificate, Case Management certification desired

 

REQUIRED SKILLS:

1. Possesses excellent written and spoken communication skills.

2. Has the ability to communicate with health care professionals. Demonstrates diplomacy, tact and a professional demeanor

3. Working knowledge of CMS and other review agency standards.

4. Strong organizational and interpersonal skills.

5. Demonstrates ability to effectively collaborate and delegate tasks.

6. Possesses ability to relate to diverse age and demographic backgrounds. .

7. Computer literate and working knowledge of software applications (word, excel).

8. Professionally appropriate analytical and organizational skills required.

 

MINIMUM PHYSICAL REQUIREMENTS:

  • Bend/twist; squat/kneel; climb stairs/ladder; reaching above shoulder; lifting/carrying 0-20lbs; pushing/pulling 0-25lbs; work environment may include contact with communicable diseases and/or pathogens/patient equipment/machinery/electricity: OCCASIONALLY - 1-33% of the time.
  • Standing/walking; driving; vision: depth perception/color vision/peripheral vision - FREQUENTLY - 34-66% of the time.
  • Sitting; flexing/extending of neck; repetitive hand/arm use; computer skills; audible speech/hearing acuity; seeing - far/near; problem solving/decision making/interpreting data - CONTINUOUSLY - 67-100% of the time.
