Overview

12 hour shifts, 3 shifts a pay period, 7:00pm-7:00am, every 4th weekend, and holiday rotation requirement

Part-Time, Non-exempt, Benefits eligible 

Minimum rate: $34/hr

 

 

The Physician Consult/Transfer Nurse is responsible for assigning patients to the appropriate unit while following the guidelines of the admitting matrix. Responsible for coordinating admissions, discharges and transfers of patients to Nationwide Children's Hospital, assist with emergency department referrals and enables access to pediatric specialist consultation.  Acts as a liaison between patient care services staff, medical staff, supervisors, and the Admitting Department and external providers to facilitate the patient admission/transfer process.

 

Demonstrates competencies defined (*) in the Physician Consult/Transfer Nurse responsibilities providing age appropriate care to patients from the age of birth through 21 years and select adults.

Responsibilities

  • Collaborate with medical staff, patient care services, and supervisor to ensure appropriate patient placement. 
  • Assigns patient to a unit according to the matrix guidelines.
  • Serves as backup for facilitation of patient End of Life transfers. 
  • Demonstrates good organizational skills
  • Utilizes positive and effective interpersonal skills.  
  • Ability to communicate clearly and effectively. 
  • Models the values of Nationwide Children's Hospital. 
  • Receives information regarding admissions in regards to condition, stability, individual needs, infectious disease exposure and communicates to appropriate personnel. 
  • Completes the pre-registration process for direct admits.  
  • Assesses the clinical status of direct admits and scheduled patients and directs them to the Emergency Department if necessary.                        
  • Assists staff on units as directed by the supervisor.  
  • Adheres to department budgetary standards.  
  • Communicates pertinent patient placement concerns to manager.  
  • Accepts and supports change in a positive manner.  
  • Participates in Patient Care Services and Hospital activities in one or more of the following: 
  • Policy/procedure review and revision.
  • Department meeting attendance and participation.
  • Committee attendance and participation.
  • Shared governance/strategic partners
  • Participates in educational activities in one or more of the following:
  • Peer review
  • Serves as a preceptor/clinical resource
  • Orientation and continuing education
  • Planning and presenting classes/inservices 
  • Participates in research and quality improvement activities and special projects.
  • Maintains professional standards by setting professional goals and participating in activities to meet goals. 
  • Facilitate transportation of patient to Nationwide Children's Hospital. 
  • Facilitate physician to physician consultation and complete documentation of information in patient medical record. 
  • Facilitates calls requesting telemedicine consults with Nationwide Children's Hospital physicians. 
  • During specified hours, receives calls and completes phone triage with Nationwide Children's Hospital Primary Care Patient for guidance on care needs. 
  • Performs other duties as assigned. 
  • Practices professional nursing in accordance with the ANA Code of Ethics for Nurses. 

Qualifications

Knowledge, Skills and Abilities Required

1.    Education:  Graduate of approved school of nursing. 

2.    Licensure:  Licensed as a Registered Nurse in the State of Ohio. 

3.    Pediatric clinical experience required. 

4.    Effective communication skills for relating to all levels of personnel and contacts. 

5.    3-5 years of clinical experience required. 

6.    Excellent verbal and written communication skills. 

7.    Familiarity with personal computer and/or main frame systems required.  Skill in using databases and Microsoft Office highly preferred. 

8.    Must be able to type on a computer keyboard and manage use of telephone equipment. 

9.    Excellent interpersonal and critical thinking skills.
