    Clinical Social Worker – Partner For Kids


    We are seeking a Clinical Social Worker– Partner For Kids. This position will be responsible for providing developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq. The Clinical Social Worker functions as a member of the interdisciplinary team and will work closely with providers, payers, and patients to improve care for children with complex medical issues in a team environment.

    What Will You Be Doing?

    • Implement interventions designed to improve health, care, and cost for PFK
    • Outreach to physicians and other service providers to identify patients in need of care coordination
    • Guide families through the health care system to optimize care and services
    • Complete and document health status assessment, plan of care and monitor set outcomes
    • Refer/connect/collaborate with community agencies to assist in and provide resources for patients
    • Demonstrate principles of palliative care and incorporate in care coordination activities


    What Are We Looking For?

    To fulfill this role successfully, you must possess these minimum qualifications and experience:

    • Social Work license in Ohio
    • Bachelor's Degree in Social Work
    • Demonstrated leadership and teaching abilities
    • Travel and home visiting within a 34 county; valid license, solid driving record and working vehicle
    • Proof of auto insurance

    • Sitting Constantly (67-100%)
    • Typing on a keyboard frequently (34-66%)
    • Standing/walking occasionally (0-33%)
