This is a Full-time position, working 40 hours per week, Day Shift, 8am-4:30pm, Weekdays only, Benefits eligible position.
Must be able to travel to all of our close to home sites; Main campus, Dublin, Westerville, Canal Winchester, and East Broad Street.
Starting minimum payrate: $23.809/hour.
The Ambulatory Care Manager emphasizes health promotion and disease prevention as they assist patients and families to move rapidly, uneventfully, safely, and as effectively as possible through their episode of care. The Ambulatory Care Manager delegates, supervises and provides patient care; plans for education and discharge; and provides for patient safety and quality of care.Demonstrates competencies defined (*) in AMBULATORY CARE MANAGER responsibilities for providing age-appropriate care to patients from the age of birth through the lifespan, and select adult patients.In addition, the Ambulatory Care Manager assists the Program Manager in supervising clinical operations, planning, evaluating and controlling program services and ensuring that internal and external standards are met.
*1. Expertly uses and serves as a resource for the nursing process, Standards of Care, clinical pathways, and clinic specific competencies, to:
a. Complete the appropriate health status assessment
b. Document a plan of care on the medical summary and/or clinic visit sheet or electronic medical record as appropriate
c. Delegate patient care as appropriate
d. Evaluate patient care and patient’s response/progress
e. Reassess patients and revise plan of care as appropriate
f. Supervise patient care
g. Plan discharge
h. Plan, provide, delegate, and evaluate patient/family education as appropriate
i. Collaborate effectively with all health care disciplines to facilitate optimal patient and family health and full potential
*2. Demonstrates awareness of and support for the principle of Family Centered Care.
*3. Communicates patient care needs/expectations/priorities to assigned Licensed Practical Nurses
(LPNs), Patient Care Assistants (PCAs), and allied health personnel.

5. Manages and oversees clinic functioning as appropriate.
*6. Effectively manages patient care through one or more of the following:
a. Delegating and coordinating care for patients

7. Provides nursing care focused on positive patient and family outcomes

8. Maintains professional standards and serves as a role model by setting professional goals and
participating in activities to meet goals.
9. Manages clinic operations within clinic budgetary standards:
a. Assists with daily staffing as appropriate
b. Analyzes data to justify and/or correct variances as appropriate
c. Participates in clinic budget preparation as appropriate
d. Participates in review and revising of visit charge sheets as appropriate
10. Provides leadership by delegating and supervising clinic maintenance activities:
a. Completion of daily visit charges
b. Infection control activities
c. CLIA maintenance as appropriate
11. Participates, delegates and enlists staff participation in Patient Care Services and Hospital activities:
a. Policy/procedure review and revision
b. Helping Hand review and revision
c. Patient/family advocacy
12. Participates in clinic, hospital meetings, and shared governance/unit council.
13. Participates & contributes to the professional development of peers and colleagues
a. Completes peer reviews
b. Serves as preceptor/resource
c. Provides and/or coordinates continuing education programs
d. Assists with clinical competency assessment.
14. Coordinates research, quality improvement activities, and special projects.
15. Participates in evidence based practice through application of policy, review of literature, and/or participation in nursing research.
16. Provides assistance to and coverage for the Program Manager.

18. Plans and coordinates program activities to meet regulatory standards in one or more of the following:

19. Performs activities related to cluster/staff management:
a. Incident report follow-up
b. Performance appraisal peer review
c. Competency assessment
20. Plans, implements and evaluates program changes.
21. Coordinates managed care activities specific to clinic.
22. Practices professional nursing in accordance with ANA Code of Ethics for Nursing.
Responsible to clinic/program leadership such as Program/Operations Manager, Service Line Administrator, etc.
Provides patient care under the direction of a physician for dependent nursing functions and in collaboration with the physician for interdependent nursing functions.
Internal: Hospital Support Departments, Medical Staff, Hospital Administration, and Patient Care Services.
External: Insurance companies, schools, community agencies, referring physician’s offices, medical equipment vendors, external support systems (off-site facilities) regulatory agencies.
KNOWLEDGE , SKILLS AND ABILITIES REQUIRED
1. EDUCATION: Graduation from an ACEN or CCNE accredited school is required, BSN highly preferred. If AD hired after Jan1, 2012, must successfully complete BSN in 5 years of start date2. LICENSURE: Licensed as a Registered Nurse in the State of Ohio.3. Three years pediatric clinical experience or equivalent combination of education/experience preferred.
AGES (S) OF PATIENTS SERVED:Provides safe and competent care for the following groups of patients; neonates, pediatric, adolescents and selected adult patients.
MINIMUM PHYSICAL REQUIREMENTSMust be able to see, hear, stand, walk, speak, read and perform manual tasks with or without accommodation, and care for oneself with little or no difficulty.Must be able to independently lift up to 25 lbs.Must be able to move or reposition patients of any weight or size with the assistance of another person (s) and/or equipment.The above list of duties is intended to describe the general nature and level of work performed by individuals assigned to this classification.
